There could be some entities far away from the origin. Try by doing a zoom fit and calculating the distance from the origin. If you get a x=3.25E35 (ie 325000000000000000000000000000000000.0), delete that entity.

Another reason could be when working with images. It is recommended to use IMAGEATTACH (Insert > Attach Raster Image) to create an xref to the JPG file instead of copying and pasting images straight from other sources.

A good practice working with images is by use IMAGEATTACH then scale them with ALIGN. It scales and rotates at the same time.
